Death certificate should also mentioned cause of death.
It can also correct after any evidence or report.
Death certificate should have date of marriage if happened and date of starting living jointly.
Birth certificate mention about last delivery date if happened. And next conception is possible date after consulting doctor and mother desire.
Please show me birth & death certificate application form for better suggestion.
Adhar card no. can also mention on birth certificate.

As for the unrecorded deaths
01 To make the eldest male of the family responsible for providing information about the death of any member, default on his side he may be fined and subsequent default be prosecuted .
02 To have the officers appointed at ward and block levels handling the above mentioned system (recieving information) and they be held accountable similarly but only on their negligence or lapse in performing their duty.

My suggestion is regarding the thousands of unrecorded births and deaths
firstly to tackle the hurdle of unrecorded births, following measures can be taken
1 aadhar card of the child should be made only on the ground of birth certificate from the muncipality.
2 of all the free vaccines provided by the government, atleast one of them should be provided only on the ground of birth certificates (probably the vaccine provided in the later years ) to have the record the previous 5-7 year.

Registration of births and deaths is very important from the legal point of view. For any government benefits, a person's birth and death details is mandatory. For a child to get admission in school, age is required. Hence every birth has to be registered in the local govt. Office . Similarly all deaths also need to be registered for any govt financial benefits accruing to the dependents. Lets encourage every citizen in the remotest area to register and a local office to help do that

Birth and death certificates are mandatory. Population control should be strictly followed, as most of the muslims are illiterate so they do many marriages and children in comparison to Hindus. They are taking major portion of help, given by the gov.in the form of food grains money jobs etc.
They are weakening our ECONOMY,STOPPING GROWTH. So Strictly regulate the policy and stop their production. Otherwise their number will be big. They will gulp the development, and progress. Think seriously.
