Birth certificate must have mentioned about how many child delivered before this.
Whether was twim or single.
Are they alive.
Mother died or alive after birth.
I have not seen your new birth and death cerificate so i can not give better suggestion.
Show me dummy birth and death certificate to give more suggestion.

The Proposed Amendments to the Registration of Births and Deaths Act, 1969 seems to be very fine. In Section 24 (2) on the payment of such sum of money, such person shall be discharged, and no further proceedings shall be taken against him in respect of such offence. Similarly no further proceedings shall be taken against institution too.
Overall the Amendments are up to the mark ,fine should be Rs.2,000/- for individuals and Rs.5,000/- for institutions or as authorities deem fit.

Birth certificate must mentioned about how child delivered. Normal/ Cesarean/ surrogacy delivery
Number of child delivered.
Institutional delivery or not
Govt hospital or private delivery
Father and mother date of birth
Weight of child & mother
Do not mention sex, boy, girl or male, female.
This is sexuality. This cause female foeticide.
Mention ONLY Mr. Mrs. Before name.
How many days/ month child lived in womb
If adoption than write adopted mother and father

Death certificate require spouse father mother name.
If spelling mistake create lots of problem.
It is not posdible at the time of death to give correct information.
I want adhar card copy should attached at श्मशान घाट office.
If old people father mother or spouse do not have adar card than spelling allow to change ssveral time.
Sms information to send before issuing death certifate about name and other detail.
So family person can sure about correctness.

यदि संभव हो सके तो कोई भी व्यक्ति अपने इच्छानुसार वास्तविक जन्म तिथि व वर्ष पुनः सही करवा सके , बीते वर्षो में स्कूल या प्रशासन की ओर से गलतियो के कारण या जागरूकता न होने के कारण तिथि गलत चलती आ रही हो और व्यक्ति अपने वास्तविक जन्मतिथि का हर्ष भी नही माना पता है । इसे बदलने के कारण आवेदक को किसी भी प्रकार का किसी भी क्षेत्र में लाभ या हानि नए नियम लागू होने के बाद न मिले क्योकि यह उसकी इच्छानुसार है ।

जिसका जन्म हुआ है उसकी मृत्यु निश्चित है। इसलिए जन्म प्रमाण पत्र के साथ ही मृत्यु प्रमाण पत्र बना दिया जाए बिना मृत्यु की तारीख डाले। जब मृत्यु यदि अस्पताल मे हो तो अस्पताल के अधिकारी से मृत्यु की तारीख की सील लगाकर हस्ताक्षर करवा लिये जाए और यदि घर पर हो तो शमशान का जो अध्यक्ष हो उससे मृत्यु की तारीख की सील लगाकर हस्ताक्षर करवा लिये जाए और online record मृत्यु की तारीख सहित update कर दिया जाए। ताकि formalities मे जो समय बरबाद होता है वो एक ही बार हो, बार बार न हो।

it should be made mandatory to submit Aadhar Card at cremation grounds of any religion in India and to be intimated to all authorities like income tax, banks, etc. to which it is linked.
